  Manager Application Form this will allow you to print out a Manager Application form. Forward this information to a Team Manager/Coach or mail it to our league mailing address.

  ANYONE planning on coaching in a Bothell Baseball League must fill out and submit a background check form. Please download, print, fill out completely, and submit this background check form. Once the entire form has been completed, please mail it to Bothell Pony for the League President's signature. It will then be forwarded to the Wahington State Patrol. Please be sure to complete Sections C and D (and sign) before sending the form.
